Deposit & Administrative Fees:

A minimum deposit of $450.00 is necessary in order to reserve a space. The $450 deposit is composed of a $300 for the resident's First & Last weeks' rent (Non-Refundable) and a $150 Security Deposit. The $150 Deposit is refundable upon the completion of the terms of the Residential Lease, subject to a $75 cleaning charge and any outstanding rent balances. The deposit is refundable solely at the discretion of Change House.

Additional Rent:

All payments are applied first to any outstanding balances and then to any current amounts due. Rent is due on a weekly basis (i.e., from Saturday to Friday) and is due on the Friday immediately preceding the following rental week. There are no partial refunds available for individuals leaving before the end of the rental week and/or prior to the full term of the rental agreement.

Resident Funds:

The Resident Funds Account is an additional service that Change House provides for our residents. All payments for the Resident Funds Account are subject to an administrative fee, which will be charged in addition to the amounts to be allocated to the individual resident's cash account. Any credits will be refunded upon the completion of the terms of the Residential Lease, subject to any necessary cleaning charges and/or outstanding rent balances.

Partial Refunds:

Any partial refunds are at the sole discretion of Change House and will be made within 30 days electronically to the card or bank account from which they originated. In the event we process a refund, a Refund Notification will be forwarded to the email address that the Registered User (Payee) entered during registration.

Acknowledgement & Acceptance of Terms:

All Internet transactions are final, and are not subject to reversal and/or cancellation. All Internet payment transactions are subject to a Convenience Fee equal to $20.00. This Convenience Fee is disclosed and included in the total payment amount prior to submission of your payment for approval. Note regarding "Save this information" and our "Express Checkout" feature: Change House will not have access to this information. Your bank account and/or card data is encrypted prior to authorization. In exchange, the secure gateway returns a "token" to this payment system which can then be resubmitted with future payments in place of more sensitive information. End-to-End Encryption coupled with Tokenization ensures your data is secure.

PREMISES

The Occupant resides in the capacity of a Lodger, sharing a housing unit and not as a tenant and therefore has no rights or possession of space exclusively. The Company grants permission to Occupant and Occupant accepts in its present condition of the room which forms part of the premises.

 

(a)  Utilities

Utilities including gas, electric, water, local phone, satellite television, computer and internet access and laundry facilities are provided, maintained and paid for by The Company and are available for Occupant's use. Occupant agrees to keep personal energy and water consumption within reasonable limits and to take all measures and make all efforts that a prudent Occupant would make to reduce wasteful overuse and to conserve energy and resources.

 

(b)   Linens

Bedroom linens are provided for Occupant and include comforter set, sheet set, pillows and protectors and with mattress pad and protector. Bathroom linens are provided and include two

(2) bath towel towels, one(!) hand towels and two (2) wash cloths.

 

(c)  Kitchen Housewares, Food and Supplies

All kitchen housewares will be provided for Occupant's use. Coffee and. filtered water will be provided and food staples such as flour, sugar, salt and cooking spices will be provided. All cleaning supplies will be provided for Occupant's use. Occupants are responsible for their own meals.

 

 Failure to perform according to the terms of this Agreement will result iimmediate removal from Premises. Occupant is required to give a THIRTY DAY (30) written notice of intention to vacate the Premises.

 

PROGRAM FEE

 (a)   Grace Period

A three (3) day grace period will be in effect to pay the Program Fee, late charges will accrue on the 4th day of the month.

 

(b)  Late Payment Charge

A late payment charge of $ 00.00 per day for each additional day that the Program Fee is unpaid up to 7 days. On the 8th day, Occupant will vacate the Premises for non-payment.

 

(c)  Returned Checks

In the event that any check is returned for insufficient funds or a closed or nonexistent account, Occupant will be charged a $25.00 returned check fee.

 

USE OF PREMISES.

The room shall be used and occupied by Occupant and shall not use the room or any part of the Premises during the term of this Agreement for the purpose of carrying on any business, profession, or trade of any kind or for any purpose other than in a lodging capacity.


ALCOHOL AND DRUG FREE ENVIRONMENT.

The Company is a transitional sober living enviromnent with zero tolerance for alcohol or drug use. This Agreement is a privilege made available to the Occupant with the strict understanding that the Premises is to be, at all times, alcohol and drug free. Any consumption of alcohol or drugs by Occupant on or off of the Premises will terminate this Agreement. Protecting and/or not disclosing to the House Manager another Occupant's alcohol or drug use will also terminate this Agreement. Upon termination, all prepaid Program Fees, Deposits and Other Fees will be forfeited.                 ยท

 

REQUIREMENTS  OF OCCUPANT

Occupant must (I) be recommended by a Clinical Director, Counselor/ Therapist, or Sponsor (2) have completed a minimum 30-Day Inpatient Treatment Program for alcohol and/or drug abuse or (3) have at least 30 days of continuous sobriety and have a Sponsor.

The ongoing requirements of the Occupant at the time of the execution of this Agreement shall include:

(a)   Occupant must have full time employment within 14 days of the execution of this Agreement or part time employment if attending an Intensive Outpatient Program (lOP), taking educational courses and/or participating in volunteer work

(b)  Attend daily recovery 12-step meetings including one (1) mandatory AA meeting on the Premises

(c)  Attend one (I) mandatory Weekly House Meeting

(d)  Have a Sponsor

(e)  Abide by all House Rules, curfews and participate in weekly chores

(f)  Keep the room they are occupying and Premises clean and safe at all times

(g)  Do not deliberately or negligently destroy, deface, damage, impair or remove any part of the Premises or knowingly permit a person to do so.

(h)  Conduct himselflherselfin a manner not to disturb any other Occupant's peaceful enjoyment

of the Premises

(i)   To respect House Manager and the policies set forth without any threat of intimidation or violence

(j) To respect all other Occupant's personal property and treat other Occupants with respect without any threat of intimidation or violence

 

RULES FOR OCCUPANTS AND SIGN IN/OUT BOARD

All House Rules governing the Occupants' use of the Premises will be posted and have been furnished to Occupant as part of this Agreement. Additionally, The Company, from time to time, may adopt additional rules or change existing rules concerning Occupants' use of the Premises. Occupant agrees to abide by such House Rules. Occupant will communicate whereabouts each day to the House Manager through updating the Sign In/Out Board regarding activities for the day.

 

OCCUPANT

Occupant agrees that the room shall be occupied by the individual identified in this Agreement. Any guest of Occupant must be approved in advance by the House Manager prior to entry onto the Premises. Approved guests are not allowed in Occupant's room nor is anyone other than the Occupant allowed to stay overnight.


SEXUAL ACTIVITY

The Premises is currently operated as a Co-Ed transitional sober living home. There is no sexual activity of any kind is allowed on the Premises at any time. Any public display of affection between Occupants or promiscuous behavior is not allowed. This is strictly enforced. Any sexual activity will terminate this Agreement and all prepaid Program Fees, Deposits and Other Fees  will be forfeited. Any pornographic material found will terminate this Agreement.

 

MEDICATION & MONITORING

All medication is stored in a locked safe with the House Manager. The House Manager will not dispense medication but will provide access to Occupant to dispense his/her own medication as prescribed.
